How to Submit a Complaint to Us
If you have a concern and would like to let us know directly, please use our online complaint form. This form helps us gather the information we need to address your concerns promptly. Once submitted, your feedback will go directly to our team, and we’ll follow up with you as soon as possible.
Filing a Formal Complaint with BHEC
If you need to file a formal complaint about professional conduct, you can do so with the Texas Behavioral Health Executive Council (BHEC). You can contact BHEC directly to file your complaint using the following information:
How Do I File a Consumer Complaint?
If you have a concern about your privacy rights or how your protected health information has been handled, we encourage you to contact Phoenix Arise first so we have the opportunity to address and resolve the issue directly.
If you believe your privacy rights have been violated, you may file a consumer complaint with one or both of the following agencies:
