Serena Shade has a Master of Science in Counseling Psychology from Angelo State University. In addition, she is pursuing her Doctoral Degree in Counselor Education from Texas Tech University. Serena has been a Licensed Professional Counselor since 2019. She is also a Licensed Chemical Dependency Counselor Intern.
Serena has been working in the field of mental health since 2010. Her first job at River Crest Hospital in San Angelo, Texas, allowed her to work with children, adolescents, adults, and veterans with various mental health issues. Serena has also worked with juvenile offenders, adult offenders, substance abusers, couples, families, and trauma victims throughout her career. She is also EMDR trained. Serena has clinical experience in working with those dealing with depression, anxiety, trauma, bipolar disorder, borderline personality disorder, and substance use disorder. In addition to individual counseling, Serena has also facilitated many group counseling sessions.
Although Serena enjoys withing with any individual who seeks to improve their mental well-being, she is genuinely passionate about her work with substance abusers. She strives to meet the individual where they are in their journey and assist them in improving their life. Serena oversees the Collegiate Mental Health Recovery Community at Texas Tech University, a program that supports collegiate students who struggle with mental illnesses. She attempts to spread the following message to as many people as possible: "it is okay not to be okay."
Serena is currently available for session by appointment and accepts Aetna, Team Choice, Baylor Scott & White, First Care, and United Healthcare. She also accepts private pay.
She sees clients at our South Lubbock location, located at 2507 79th St., Suite B.

GOOD FAITH ESTIMATE WEBSITE NOTICE
Under Section 2799B-6 of the Public Health Service Act, the “No Surprises Act,” you have a right to receive a “good faith estimate” explaining how much your therapy services will cost. Healthcare providers are required to provide clients who do not have insurance or who choose not to use their insurance for therapy services with an estimate of the cost of those services.
· You have the right to receive a Good Faith Estimate for the total cost of therapy services
· Make sure your therapist gives you a Good Faith Estimate in writing at least 1 business day before your appointment. You may also ask your therapy for a Good Faith Estimate before you schedule an appointment.
· If you receive a bill that is at least $400 more than your Good Faith Estimate you can dispute the bill.
· Make sure to save a copy or picture of your Good Faith Estimate.

Phoenix Arise is a Covered Entity and must comply with the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), the Health Information Technology for Economic and Clinical Health Act (“HITECH” or the “HITECH Act”), and Chapters 181 and 182 of the Texas Health & Safety Code. Phoenix Arise may not disclose your PHI electronically without your authorization unless allowed by law. Phoenix Arise may not disclose PHI electronically without separate authorization from you or your personal representative for each disclosure unless the disclosure is for treatment, payment, health care operations, an insurance or HMO function, or as otherwise authorized under state or federal law.
